abstract
This form accompanies Peripheral’s Guide for Reviewers, and is used by reviewers for Peripherals to record their assessment of a manuscript. Parts 1 to 8 reach the author with the reviewer's name removed, and comprise a summary, a ratings grid, an assessment, and essential and strengthening revisions. Parts 0 and 9 are seen only by the editors, holding declarations of conflict, competence and confidentiality together with the recommendation on publication. Those declarations and the routes for reporting ethics and permissions concerns follow the COPE Ethical Guidelines for Peer Reviewers and the COPE Core Practices. The form is issued as a printable document and a digitally fillable template.
